Scheduling

Items to have on-hand when calling are:  the children's social security number, date of birth, and, if possible, the Medicaid number. 

If you don't have the child’s Medicaid number, we can verify him or her by the date of birth, name and Social Security number.

(Youth Bridge receives funding from a variety of sources.  If you do not qualify for Medicaid, do not be concerned.  We will NOT turn a child away based on financial reasons.)

Intake & Assessment

When attending the appointment, please make sure that the adult accompanying the child is the legal guardian and has the authority to sign for the child to receive services. This program is for young adults - no guardian is necessary.

At the appointment you need to bring a Social Security card (If you can't find it, you can bring a copy of your tax return to verify the youth's social security # and/or a copy of the birth certificate).  We also need to make a copy of their Medicaid Card, if necessary.

During the appointment, you and your child will meet with Youth Bridge staff to assess his or her needs.  Our staff will then form a treatment plan based upon this meeting.  You will be notified shortly thereafter and receive a copy of the treatment plan.  Once our licensed psychiatrist has signed off on the plan and the parent or guardian has approved it, treatment can begin.
